Terms & Conditions

Please read thoroughly and carefully before signing and submitting your registration form. 

 

IN CONSIDERATION of allowing my minor child/ward to participate and / or volunteer at The After School Spot. The term “TASS” shall refer to the organization participating in, sponsoring, or administering to the youth volunteer as well as any and all TASS members, and staff. I agree and acknowledge that: 

 

1. I am the parent /guardian having full legal responsibility for the minor/child. 

 

2. I am aware and familiar with the fact that TASS is a nonprofit snack shop and lounge for middle and high school kids located in the City of Sebring. 

 

3. I will abide by the rules and regulations listed in this document. 

 

4. I freely and voluntarily assume any risks and hazards inherent in the nature of volunteering at TASS and accordingly my participation shall be entirely at my own risk. 

 

5. I waive any claim I may have against TASS arising from my volunteerism, and agree to indemnify and save harmless TASS from any claim, including any claim for medical service arising from my participation in volunteering. 

 

6. TASS may secure such medical advice and services as it, in its sole discretion, may deem necessary for my health and safety and I shall be financially responsible for such advice and services. 

 

7. The release, waiver of claim, and assumption of risk is binding on myself, my heirs, my executors, administrators, personal representatives, parents/guardians, and assignees. 

 

8. I also release TASS from any and all liability resulting from my verbal testimony given of my own free will. 

 

9. My child/ward has read this information with me, and anything not understood has been clarified for them. 

Additional Rules & Safety / Boundaries 

Volunteers and employees should conduct themselves in a professional manner exercising prudent judgment, diligence in the performance of their duties, and agree to the following boundaries: 

 

1. I will not give out any unnecessary personal information about myself (including last name, address, phone number, email address, place of employment, etc.). 

 

2. I will not disclose any confidential business information I learn while at TASS.I will not take photos or share stories with the media without the full permission of the individuals involved. 

 

3. I will not accept or offer gifts. 

 

4. I will not go into an isolated area of the TASS building or property by myself or without permission. I will make sure there is always another volunteer or a staff member present. 

 

5. I will attend volunteer meetings to stay updated on policy.

 

6. I will be very cautious regarding physical touch with other students, and staff and will not engage in any kind of touch (hugging, arm around the shoulder, etc.) unless I’ve ensured that both parties are comfortable with this. 

 

7. I will not get involved if there is a crisis situation at TASS, but will alert staff and step back to a safe distance. 

 

8. I will dress appropriately while I am volunteering at TASS.

 

9. I will refer individuals to TASS staff if they ask me something that I am unsure how to answer. If I am ever in a situation where I do not feel comfortable, I will seek the assistance of a TASS staff member immediately. 

 

10. I understand that TASS is not responsible for any personal belongings, and I will leave expensive valuables at home or in my backpack. 

 

11. I understand that I am expected to arrive on time and stay for the full length of my shift, as the rest of the team is relying on my participation. If I can’t make my shift for whatever reason, I understand that I am expected to email or phone the volunteer coordinator or department supervisor with as much notice as possible or complete a shift swap with another volunteer. I understand that volunteers that miss two shifts without notice/shift swap will be removed from the volunteer pool. 

 

12. I will sign in at the beginning of each shift at TASS. I am aware that TASS tracks volunteer hours for statistical purposes. 

 

13. I will not use derogatory or profane language in TASS, or play music or media with such, at any time.

 

14.The After School Spot use of photos taken for promotional material or social media along with the printing of first names has been accepted or rejected on initial registration from and no further photo releases shall be necessary.

 

As the parent/legal guardian you have read and understood the above terms and conditions, waivers and attached rules & safety and do hereby indicate the approval of your child(ren)s participation in becoming a participant at The After School Spot in Sebring, Florida.

The heart of our work lies in the continuous passion and dedication we bring to this project and the range of future classes we plan to add to our center. Our core value is interdependency, where we strive to demonstrate collaboration over competition.
